Testing Request Form

  1. Read the Sales Terms and Conditions document.
  2. Download the Testing Request Form document.
  3. Complete the Testing Request Form.
  4. Ensure that the appropriate sample size is provided(see chart below).
  5. E-mail all testing requests to testing.request@precilab.com.
  6. A copy of the MSDS (electronic preferred) is required for first submissions of all non-standard semiconductor materials. No work can be started without the MSDS.
